Customize your QuickStart by including questions you'd like to go over during your intake session. We understand that you've got your go-to questions and like the information they provide, so we want to give you the opportunity to get even more thoughtful answers from your new clients. 
 
When you complete your purchase, you will be given a form to include your self-assessment. Please be thourough and write it how you'd like it to be structured. Once you're finished with the form and send it to YourEMDRGuide@gmail.com, our team will turn it into a fully fillable PDF that you can send to clients before their first session!
